Our plan will provide you with 24/7 telephone access to a clinician, continuity of care for services that you are already receiving, and a wide range of services.
Services
RiverSpring Star (HMO I-SNP) will provide all your covered Medicare services, including:
- Ambulance, chiropractic care, diabetic supplies, diagnostic tests, lab and radiology services, x-rays, doctor’s office visits, durable medical equipment (DME), emergency care, foot care, home health care, mental health care, outpatient rehabilitation, outpatient substance abuse, outpatient surgery, prosthetic devices, renal dialysis, urgently needed care, preventative care, skilled nursing facility care, and much more.
Costs

Plan Premium $48.70 per month
In addition, you must keep paying your Medicare Part B Premium
This plan has deductibles for some hospital and medical services, and Part D prescription drugs.
$545 per year for Part D prescription drugs.
$240 per year for Part B Deductible for in-network services.
Your cost sharing may be less depending on certain circumstances
